Self Management
The treatment, support and empowerment of people with long-term conditions (LTC) are key issues within the NHS as outlined in domain 2 of ‘Liberating the NHS; Transparency in Outcomes – a framework for the NHS’ (‘Enhancing quality of life for people with long-term conditions’)
The subject of ‘long-term conditions’ also forms one of the twelve workstreams set up by the DH to help manage the delivery of Quality, Innovation, Productivity and Prevention agenda (QIPP) in the NHS. Click here for more details.
One of the 4 key principles of the LTC workstream is;
‘Empowering patients to maximise self-management including ensuring patients have appropriate information and knowledge about how to manage their condition’.
Our workshops have been designed to help health professionals explore in detail the interpersonal skills involved necessary to support and best enable patients to manage their condition. It has been shown that the better the working partnership with a patient the greater the confidence the patient has in self management.
CFEP has been proud to be a member of the National Support Team for The Health Foundation's Co Creating Health Initiative during Phase 1 of the project, providing the Advanced Development Programme for clinicians. Our workshops have also played a key part in the Supporting Self Care project for the East of England SHA, the Macmillan National Cancer Survivorship Initiative and as part of commissioned work targeting teams of community based Diabetes Specialist Nurses.
In addition, by carrying out a workshop in conjunction with CFEP’s questionnaire ‘Patient Partnership in Care (PPiC)’ practitioners have been enabled to identify and fine tune specific interpersonal skills which better equip patients in self management.
